How to register

Joining The Practice

  • Anyone living in our practice area can join
    Regardless of age, personal circumstances or medical problems
  • As a patient you can choose to see any of the doctors
    However, we recommend that you try to establish a relationship with one doctor, who you come to regard as your usual doctor, to aid continuity and quality of care
  • We are a non-limited partnership NHS practice
    Contracted to provide general medical services by NHS England commissioning board.
  • We register anyone eligible for NHS treatment who is living in our catchment area
    Or extended boundary (see map below). We would be delighted to register all persons or family members living at your address in order to offer full care.

When registering with the practice please complete all relevant forms (included below), print them off, and sign them before returning them to the surgery (in person or by post) with proof of address and ID. Photocopies are acceptable.

We will check the forms at the surgery, and we will then register you, if you have your NHS number then please include this in the registration form.

Please bring your NHS medical card to register, if you do not have your NHS medical card, you can register by filling out your details on a registration form but it helps if you have the name and address of your previous NHS doctor and your national health service number.

We will require you to have two proofs of ID, one of which should be a photo ID (e.g. Passport, driver’s license, or documentation from the home office), and the other can be a current utility bill, council tax statement, or tenancy agreement. If you are unable to supply these, please discuss further with our reception team.

Catchment Area


Moving Home?

If you move home outside our catchment area you will need to re-register with a GP local to where you live.

We are not able to provide general medical services, including home visits, once you have moved outside our catchment area.

Registration for university students

It is essential that all students register with a local doctor’s surgery when they move into their new accommodation, whether it be a Hall of Residence, house or flat. This will provide you with easy access to the full range of medical services during your time at University.
